USA Softball offers supplemental insurance. In the past - each team was required to purchase insurance for at least 10 players. For the 2025 season we have opted to remove insurance from team dues and let players sign up individually.

Here is what the insurance covers per USA Softball. We encourage everyone to take advantage of this supplemental insurance as it would help minimize out of pocket expenses if you are injured while playing softball. Coverage would be for the 2025 season only.

For coverage prior to Ryan Kelley Classic - you must purchase by end of day 3/17/2025. The final cutoff for the Spring/Summer TSRA season will be end of day 3/30/2024.

ACCIDENT POLICY

The USA Softball Accident plan provides coverage for medical expenses that you incur as a result of your participating in supervised USA Softball Team activities. This is a supplemental policy to help minimize your out of pocket expenses if you are injured playing softball. The coverage is subject to limits, conditions and exclusions of the policy and is not designed to cover "everything.”

Please note: It is very important that you follow your primary insurance carrier’s eligibility criteria (e.g., to be treated in-network, if required by HMO, etc.) in order for this policy to consider your expenses for payment.

Policy Limits Accident Medical $250,000 per claim

Accidental Death/Dismemberment $10,000Individual

Team Insurance Deductible $1,000 with 90/10 coinsurance

Claim Guidelines

  • Medical services must begin within thirty days of the accident

  • Claim forms must be submitted within ninety days or up to one year from the date of injury

  • The policy is subject to a 52 week benefit period from the day of injury

  • Payment is made according to usual and customary guidelines

Additional benefits and conditions of coverage:

a. Eligible expenses for treatment are covered for 52 weeks from the date of injury. Any expenses incurred

beyond the benefit period are not covered by this policy.

b. 90/10 Coinsurance.

c. Medical and Dental services must begin within 60 days from the date of injury.

d. Claim must be submitted to RPS Bollinger up to one year from the date of injury to be eligible for payment.

e. Deductible is a “corridor” deductible, which applies regardless of payments by other primary insurance.

f. Claims are paid based on a Usual and Customary Basis which means Expenses (a) charged for treatment, supplies or medical services

which are Medically Necessary to treat the Insured’s condition; and (b) which do not exceed the usual level of charges made for similar

treatment, supplies or medical services in the locality where incurred.

g. Physical Therapy/Chiropractic limit: $2,500 limit per injury, limited to $100 maximum per visit

h. Durable Medical Equipment limit: $1,000 per injury

i. Prescription Drug Limit: $1,000 per injury
